The care involved extensive education for the patients and their families. Our social workers ensure that patients and their families access relevant community resources.

Here’s how each member of the team can assist you:

• Doctors and Nurses: Provide education on the proper use of durable medical equipment and medical supplies, education for proper symptom management, emergency planning, proper medication administration, how to properly carry out the patient’s care plan, and essential safety tips.
• Social Workers and Bereavement Counselors: Provide education on advanced directives, give bereavement counseling, help with mortuary planning, and provide coping strategies and tools.
• Spiritual Counselors: Provide spiritual guidance, offer reflective listening and non-judgmental emotional/spiritual comfort, help to make a connection with a patient’s personal clergy and/or community of faith, and help to provide specific religious clergy and/or ritual support.
• Health Aides: Helps with proper hygiene and grooming education to prevent infections and wounds and gives essential safety tips
