You Can Transfer. Here’s What That Means.
Under Medicare and Medi-Cal, hospice patients have the right to change their hospice provider at any time — for any reason. A transfer does not affect your hospice benefit or restart any waiting periods.
Common reasons families consider a transfer to Orange Hospice:

- Feeling unsupported or left without answers

- A desire for a more personalized, community-connected team
- A recommendation from a hospital, physician, or family friend

How the Transfer Process Works
We’ve made this as simple as possible so your family can focus on what matters most.
Step 1 — Reach out to us. Complete the transfer request form below or call us directly at (714) 790-0594. Our team is available 24/7.
Step 2 — We verify your coverage. We’ll confirm your Medicare, Medi-Cal, or insurance information so there are no surprises.
Step 3 — We coordinate with your current provider. You do not need to contact your current hospice. We handle the transition on your behalf, including notifying them of the discharge date.
Step 4 — We meet your loved one. Once the transfer is complete, an Orange Hospice nurse will be at your side to assess needs, introduce the care team, and ensure continuity of all medications, equipment, and supplies.
