- Make routine visits on the agreed-upon schedule
- Respond to PRN (as-needed) visits for urgent needs
- Provide skilled nursing, psychosocial, and spiritual support consistent with the patient’s existing plan of care
- Communicate directly with the primary hospice to ensure accurate documentation and smooth coordination of services
